Second bounded replacement for WI-386. Depends on 019fe0f0-421b-7b02-9918-513cf2ad40c8.

Add the immutable graph-version pin to Work Items at entry/promotion, backfill existing live items deterministically, and route scheduler, exit-bar, review, transition, and UI-detail reads through that pin. New items use the workflow's current graph version; in-flight items never silently repin.

Acceptance:

Rewire the current workflow while an item is mid-flight: the old item still resolves its original phases and a new item resolves the new graph.
A restart does not change either resolution.
Missing or inconsistent pins park with an explicit operator reason rather than falling back to the newest graph.

Keep counter migration and the full rewire demo for the next item.
