Anima work item 019fe0f0-4329-7843-8fe6-90dea8cfae37.

Final bounded replacement for WI-386. Depends on 019fe0f0-421b-7b02-9918-513cf2ad40c8 and 019fe0f0-42a3-72b2-bc4a-7e49e65ec241.

Key per-(Work Item, phase, graph-version) revisit/progress counters so a graph edit cannot reset safety policy. Complete the operator rewire path and an end-to-end test/demo:

  1. start an item on version A;
  2. publish version B with changed wiring;
  3. show the live item finishes on A;
  4. show a new item enters B;
  5. show counters and witnesses survive;
  6. show deleting an in-use phase is rejected while retiring it succeeds.

Do not broaden into the revisit-cap policy itself; this item supplies its stable identity substrate.
